Community Options, Inc.  is a national non-profit agency providing services to individuals with disabilities in 12 states.

The Certified Investigator conducts all investigations related to individual abuse, incidents, and other
agency affairs as needed by the agency within the state required timeframes.

The starting rate is $28.00-$30.00 per hour!

Locations:  Queens, Brooklyn, Manhattan, NY can work out of either location

Responsibilities

  • Conduct in person and virtual interviews and collect written statements and other supporting documentation pertinent to investigation

  • Act as the point of contact for investigations and coordinate the investigation process,
    collecting information requested by the investigator, schedule staff interviews, and visit
    homes/programs
     
  • Submit investigative reports in a timely manner, and corrective action plans according to the Justice Center requirements

  •  Conduct agency-wide programmatic internal audits and ensure that the agency is prepared for external audits
     
  • In conjunction with management, develop and implement corrective action plans in response to external and internal audits

  • Review investigation recommendations with the QA/Incident Management Committee andrespond to reports in a timely manner

  • Initiate and monitor the implementation of all QA/Incident Management Committee
    recommendations and report any issues

  • Ensure the agency is in compliance with all federal, state, and local regulations

  • Assist agency programs in preparing for all external audits from agencies
     Interface as an agency representative with law enforcement, state governing, and regulatory
    bodies

  •  Navigate and submit information into statewide incident database within regulatory time
    frames
     Identify areas of concern and work with Department/Program Directors to create systems to
    address these concerns
    Develop written reports of internal management audit findings if rewquired
  • Assist with incident tracking, monitoring, and trend analysis as needed 
  • Complete state approved Incident Investigation Certification Program and maintain certification as investigator 
  • Present investigation to Incident Review Committee and follow up on recommendations as
    requested

  •  Assist with curriculum and conduct agency-wide trainings on abuse prevention, incident
    reporting and other regulatory/core training topics as needed

  • Maintain all documentation securely in files electronically and in HIPAA compliant manner

  • Attend staff meetings as determined by supervisor

  •  Attend conferences, seminars. and training relevant to the position

  • Employee must cooperate with the license and department staff in any inspection, inquiry, or investigation Employment is contingent upon successful completion of checks of criminal background,
    central registry, child abuse registry, and drug testing
Requirements
  • Must be a Certified Investigator
  •  Bachelor’s degree in a related field or equivalent, and three years related experience OR any equivalent combination of experience and training

  • Valid driver’s license with satisfactory driving record

  • Commitment to and knowledge of community-based support for persons with disabilities

  • Working knowledge of management of an effective QA program, including training, monitoring, conducting, and documenting investigations, addressing violations, and monitoring corrective actions

  • Ability to maintain effective relationships with other management staff, employees,
    regulatory/oversight agencies, and the general public

  • Knowledge and understanding of local regulatory agency operations
