We are Now Hiring a Senior Community Integration Counselor to provide person-centered care tailored to meet the needs of individuals with intellectual and developmental disabilities. The Community Integration Counselor provides individualized services designed to assist participants in managing and overcoming the difficulties confronted when living in the community. This is a fantastic opportunity for professional growth, while making a difference! No experience required– we provide PAID training!
Starting pay rate is: $17.70 an hour
Shift schedule: Monday- Friday 8am- 4pm
Responsibilities
Assist individuals with daily living skills and the development of community involvement, communication skills, problem-solving skills, and social skills
Provide ongoing support for community integration efforts
Assist with developing a schedule for each participant to maximize community integration
Provide training to individuals towards achieving program goals
Complete training to facilitate staff development
Administer medications and ensure proper completion of necessary documentation
Plan, develop, facilitate, and participate in individual and group activities with participants,
Including leisure and volunteer opportunities
Develop individual day program support plans
Provide reports on individual progress in relation to overall goals
Maintain inventory and ensures adequate supplies for program
Oversee consumer funds as needed
Monitor the health and medical needs of individuals and immediately report any concerns
Drive individuals to medical appointments and activities in the community
Ensure program documentation and billable records are completed accurately and timely
Build a support network for individuals by developing and maintaining relationships with community members, businesses, and families
Employees must cooperate with the licensee and department staff in any inspection, inquiry or investigation
Additional tasks and responsibilities may be assigned
Minimum Requirements
High School Diploma or GED
Bachelor’s Degree preferred
Valid driver’s license with a satisfactory driving record
Minimum 2 years of relevant experience supporting individuals with intellectual and developmental disabilities
Ability to establish a comfortable and supportive relationship with individuals receiving supports
Ability to exercise good judgment and remain calm in crisis situations
Excellent verbal and written communication skills
Excellent time management skills
Working Conditions
Transport individuals utilizing your own vehicle or company provided vehicles
Schedule may change due to business needs and may include evening and weekend hours
May be required to be on-call in cooperation with other management staff
Frequent lifting, stretching, and other physical exertion may be required
May be required to lift or move 25+ pounds
May be exposed to various medical conditions and communicable diseases
